FAX a completed application which must be approved by the current membership. After the approval process is complete (this takes five business days) and after we receive your original, signed application and joining fee, you will be added to our contracts.

2. Go to the location where you saved the file and open it.

3. Complete and sign the application.

4. FAX the application to FSANA at (425) 458-9054.

5. Mail a signed application and a check for the joining fee to FSANA.
